Ualbany Required Agreements

All contracts, licensing forms and agreements must be submitted to the Office of State Funded Purchasing and Contracts. After review by the contract agent, they are forwarded to the appropriate signatory and, if necessary, returned to the service or contractor. Fulton-Montgomery Community College has inter-transfer contracts with several public and private universities. A transfer contract is a detailed sequence of course work approved in advance by the transfer institution. This will allow students to maximize the number of credits transferred to the four-year university in which they wish to participate. Contracts may also be required in certain situations, regardless of value, when: a) more than one payment is made to a contractor; b) where the tasks performed by the contractor are not appropriate for a regular order for the description; and (c) when the contractor imposes his contract on the university. Data use agreements describe how data is used and determine who can receive or use the data. Often, a senior auditor, teacher, staff member or student must apply for permission to access the data as part of their education or research requirements. In case of measles, also known as Rubeola – 2 doses of live measles vaccine administered after 12 months and at least 30 days apart (precise vaccination data are required) or results of a blood test indicating that you are immune to measles (copy of the laboratory report must be provided). In the case of mumps, 1 live dose of mumps vaccine given after 12 months (exact vaccination data required) or blood test results showing that you are immune to mumps (copy of the laboratory report must be provided). For rubella, also known as German measles — 1 live-dose rubella vaccine administered after 12 months (precise vaccination data are needed) or results from a blood test showing that you are immune to rubella (copy of the laboratory report must be provided). We accept one of the following documents as proof of your MMR immunity: a) A copy of your vaccinations (including updated data) on an official government/school letterhead – the easiest place to obtain it may be from your last high school or college; or b) a copy of your vaccinations (including up-to-date data) on the doctor`s header, containing the printed name, address and phone number; Or c) Leave a blood test to confirm immunity. Please note that a copy of the laboratory report must be filed.
